Eligibility:  The Blaze Pizza Sleigh Your Cravings Sweepstakes (hereinafter, the “Promotion”) is open only to individuals who are legal residents of the United States (excluding Rhode Island, Puerto Rico, U.S. territories and possessions) and who are at least eighteen (18) years old as of November 22, 2023 (hereinafter, “you” or “Entrant”). The Promotion is subject to all applicable federal, state and local laws and regulations and is void where prohibited by law.

Timing: The entry period for this Promotion shall commence on November 22, 2023 at 6:00 a.m. Pacific Daylight Time and end on December 31, 2023 at 11:59 p.m. Pacific Daylight Time (“Promotion Period”).  By participating in the Promotion, each Entrant unconditionally accepts and agrees to comply with and abide by these “Official Rules” and agrees that the decisions of Sponsor and Administrator, including the interpretation of these Official Rules, will be final and binding in all respects.

Overview: Eligible individuals may enter the Promotion by completing Sweepstakes tasks throughout the Promotion Period, as highlighted via emails sent to Blaze Rewards members. Individuals must have a Blaze Rewards account and be opted-in to receive marketing communications via Email from Blaze Pizza to receive access to instructions and participation. The Promotion Administrator will randomly select three (3) winners from qualified entries during the Promotion Period on or about the dates specified under Prizes.

How to Enter:  To participate and enter this Promotion, individuals will need to access the Internet through either a computer or a mobile phone that uses either the iPhone or Android platform.  Additionally, individuals will need to have a Blaze Rewards Account (“Account”). Successful completion of a Blaze Pizza Order while logged into Account on blazepizza.com or the Blaze Pizza App, or successfully scanning Account’s QR Code at cash register after order completion (herein after, a “Check-In”) are possible entry mechansims. Sponsor will not review or consider entries that fail to strictly follow the entry instructions and guidelines set forth in these Official Rules and neither Sponsor nor Administrator is responsible for failure to receive or view an entry regardless of the reason.  For the purpose of this Promotion, a submission is an entry that strictly adheres to the technical and legal requirements disclosed in these Official Rules, including, without limitation, the Guidelines (defined below).

How to enter during the Promotion Period:

(1)   Entry Via Blaze Rewards Check-Ins: Internet access required*. During the Promotion Period, eligible entrants will receive one (1) entry for every successful Check-In at participating Blaze Pizza locations. Eligible Check-Ins must be complete by 11:59:59 PM PST on December 31, 2023.

Entry Multiplier: Throughout the Promotion Period, the Sponsor may announce opportunities to increase the number of automatic Check-In entries you earn by a multiplier, e.g., (2X), (3X), (5X), (10X) (each an “Entry Multiplier”). When an Entry Multiplier is offered, entries will be multiplied by applicable Entry Multiplier. For example, a Check-In during a 10X Entry Multiplier offer will earn 10 entries (1x10=10). Timing for an Entry Multiplier offer will be determined solely by the Sponsor and will be announced by the Sponsor via email communications to Blaze Rewards members and/or its social media pages, website, or other domains. When a Multiplier is offered, trivia entries will earn 10 entries (these trivia entries are not subject to Entry Multipliers).

Bonus Entry: Throughout the Promotion Period, the Sponsor may announce opportunities to earn other Check-In bonus entries. Example: Earn 15 bonus entries with a Check-In that includes an 11” Pizza on Order.  Timing for a Bonus Entry Period will be determined solely by the Sponsor and will be announced by the Sponsor via email communications to Blaze Rewards members and/or its social media pages, website, or other domains. To receive additional entries during a Bonus offer without making a purchase, follow the instructions for trivia entries above.

*Normal internet access, phone, and usage charges imposed by your online or phone service may apply. If you are entering via a mobile device and using your wireless carrier’s network, standard data charges from your wireless carrier may apply. 

(2)  Entry Via Blaze Rewards Trivia Questions: Internet access required*. To receive one (1) entry without making any kind of purchase or payment, correctly answer one of the trivia questions during the Promotion Period. Trivia questions will be made available to Blaze Rewards members via email during the Promotion Period. Entrants may earn multiple entries via trivia entries throughout the month; one per trivia question answered correctly. Entrant must provide an email address associated with an active Blaze Rewards account with trivia entry for entry to be deemed eligible for selection. Limit one entry per trivia question per entrant, unless otherwise specified by an Entry Multiplier.

Limit:  You may earn unlimited entrees during the Promotion Period.  Submissions generated by a script, computer program, macro, programmed, robotic or other automated means will be disqualified.  Submissions that are incomplete, illegible, corrupted, false, lost, late or misdirected, deceptive or otherwise not in compliance with the Official Rules may be disqualified from the Promotion at the sole and absolute discretion of Sponsor and/or Administrator. 

Mobile Disclosure:  If you opt to use your wireless mobile device in connection with the Promotion, depending on your device’s capabilities, standard text messaging rates or data charges may apply according to the terms and conditions of your service agreement with your wireless carrier.  Other charges may apply (such as normal airtime and carrier charges) and may appear on your mobile phone bill or be deducted from your prepaid account balance.  Wireless carriers’ rate plans may vary, and you should contact your wireless carrier for more information on messaging rate plans and charges arising from your participation in this Promotion.

Selection of Winners [RANDOM DRAWING]: A random drawing to select three (3) winners will be conducted on or about January 3, 2023 from among all eligible Check-In and Trivia entries received. The random drawing will be conducted via an automated random selection generator, an independent tool whose decisions are final. Odds of winning a prize will depend solely on the number of eligible entries received.

Winner Notifications: The potential winners will be contacted by email and/or phone by the Sponsor. Failure to respond to a notification within the time specified will result in disqualification without further notice and an alternate winner will be selected. In the event a potential winner is deemed ineligible, fails to comply with the Official Rules or is unable to claim the prize as specified, the potential winner will be disqualified, and an alternate winner will be selected. Prize award is contingent on eligibility verification as outlined in the Official Rules. If Sponsor, or Administrator acting on Sponsor’s behalf, is unable to determine and verify a potential winner after repeated alternated drawings, or if they fail to receive a sufficient number of entries to correspond to the number of prizes(s) available to be awarded, Sponsor reserves the right to not award any or all of the prize(s).

SCAM ALERT – Protect yourself from scams. If you are selected as a potential winner in one of our Promotions, the Sponsor or Administrator will contact via the phone number and/or email address you provided on your Blaze Rewards account. You will NEVER be asked to provide a credit card, bank information, or a payment of any kind as a condition of a prize award. Those are sure signs of Promotion scams. We do NOT use social media to contact winners of the Promotion.

Prizes and Approximate Retail Value (“ARV”), Prize Value Stated in U.S. Dollars (USD): Three (3) Grand Prizes – “1 Year of Free Pizza”; distributed via a $520 Blaze Pizza Gift Card valid at U.S. Blaze Pizza locations. Prize consists of only the item specifically listed as part of the Prize. Winner is solely responsible for any federal, state and local taxes and all fees and expenses related to the acceptance of prize not specified as paid by Sponsor herein, including Blaze Pizza items totaling more than the amount made available through the prize, surcharges, service charges, or charges and expenses associated with travel required to utilize the prize. In the event winner is unable to take delivery of the prize, or is found to be ineligible for any reason, winner will be disqualified, and an alternate winner may be selected. Sponsor reserves the right to provide a prize of equal or greater value (where lawful), at its sole discretion. Prize cannot be substituted, assigned, transferred or redeemed for cash; however, Sponsor reserves the right to make equivalent prize substitutions at Sponsor’s discretion. Sponsor is not responsible for any delay or cancellation of the prize distribution due to unforeseen circumstances or those outside the Sponsor’s control. Only the number of prizes stated in the Official Rules is available to be won.

General Rules for Blaze Pizza, LLC Sponsored Promotions, Sweepstakes and Contests

Conflict Between Rules:  These General Rules shall apply to all Promotion and contests sponsored by Blaze Pizza, LLC (each, a “Promotion”) unless expressly modified or superseded by the rules written and published for a specific Promotion (the “Specific Rules”).  In the event that any provision of the Specific Rules conflicts with any provision of the General Rules provided herein, the terms of the Specific Rules shall prevail with respect to such matter.  The General Rules, along with the Specific Rules, will be referred to collectively as the “Official Rules” for the Promotion.

Interpretation of Rules: The Sponsor reserves the right in its sole discretion to interpret the Official Rules of the Promotion, and such interpretation shall be binding upon all Entrants.

Entries: No illegible, incomplete, forged or altered entries will be accepted.  All entries become the exclusive property of the Sponsor and will not be returned.  Any entry, communication or information transmitted to Sponsor by electronic mail or otherwise is and will be treated as non-confidential and nonproprietary.  Any person who attempts to participate with multiple email and/or street addresses, under multiple identities or uses any device or artifice to submit multiple entries in violation of the entry limitation will be disqualified.  In case of dispute, an online entry will be deemed made by the authorized account holder of the email address used to submit entry. “Authorized account holder” is defined as the natural person who is assigned to an address or an email address by an Internet access provider, on-line service provider, or other organization that is responsible for assigning addresses or email addresses for the domain associated with the submitted email address.  Proof of Submission of an entry form does not constitute proof of receipt.

Selection of Winners: Winners of this promotion will be determined from all eligible entries as soon as practicable throughout the duration of the promotion by promotion Administrator, whose decisions are final on all matters relating to the Promotion.  Winners will be notified via email. In the event that a promotion or sweepstakes has multiple prize periods, Entrants may enter more than one prize period, but winners will not be eligible to win prizes in subsequent prize periods, with the exception of the Grand Prize drawing, and non-winning entries will NOT be carried forward to subsequent prize period drawings.  Winners of a contest will be selected according to the criteria set forth in the Specific Rules applicable to the Promotion.

Notification and Requirements of Potential Winners:  Sponsor will attempt to notify potential winners as soon as they have been identified at random.  If a potential winner does not respond within one (1) business day after the notice is sent, the Sponsor may select an alternate potential winner in his/her place at random from all remaining entries received.  Except where prohibited, a potential winner may be required to complete and return an affidavit of eligibility and liability/publicity release.  All participants must be 18 years of age or older at the commencement of the Promotion.  If a potential winner fails to sign and return any documents within the required time period, an alternate potential winner may be selected in his/her place in a random drawing of all remaining entries received. 

Prizes and Odds:  In the event of a sweepstakes promotion (where winners are chosen by luck), the odds of winning depend upon the total number of eligible entries received.  All prizes in any contest, promotion or sweepstakes will be awarded (assuming eligible entries are received), if feasible.  Sponsor reserves the right to substitute prizes of equal or greater value.  Any prize details not specified in the Specific Rules will be determined by Sponsor in its sole discretion.  A prize may not be transferred and must be accepted as awarded.  Entrant may not request cash or a substitute prize in lieu of the prize awarded to them by Sponsor.

Prize Delivery.  Taxes and fees, if any, are the sole responsibility of prize winners. No substitution or transfer of prizes is permitted and there is no cash alternative. Prize cannot be exchanged or refunded or sold for commercial gain.  Sponsor shall not be responsible for prize utility, quality or otherwise.  All prizes are awarded “as is” without warranty of any kind or type.

Publicity/Indemnity/Disclaimers//Termination.  By accepting a Prize, to the fullest extent permitted by applicable law, the winner irrevocably grants to the Sponsor and each of their licensees, and its and their successors, assigns and sub-licensees the right and permission to use winner’s name, voice, likeness and/or biographical material for advertising, promotional and/or publicity purposes in connection with the Promotion, in all forms of media and by all manners (now and hereafter known), and on and in connection with related products, services, advertising and promotional materials (now known or hereafter developed), worldwide, in perpetuity, without any obligation, notice or monetary or other form of consideration except for the awarding of the Prize to the winner.  Winners agree not to make any publicity references concerning the Sponsor without the Sponsor’s prior written consent.

In the event Sponsor is prevented from continuing with the Promotion by any event beyond its control, including, but not limited to, fire, flood, epidemic, earthquake, explosion, labor dispute or strike, act of God or public enemy, communications or equipment failure, utility or service interruptions, riot or civil disturbance, terrorist threat or activity, war (declared or undeclared), interference with the Promotion by any party, or any federal state or local government law, order, or regulation, order of any court or jurisdiction, or other cause not reasonably within Sponsor’s control (each a “Force Majeure” event or occurrence), Sponsor shall have the right to modify, suspend or terminate the Promotion or Prize. 

Sponsor additionally reserves the right, in its sole and absolute discretion: (1) to modify, suspend or terminate the Promotion should causes beyond Sponsor’s control corrupt or interfere with the administration, integrity, operation, security or proper play of the Promotion; or (2) to disqualify any Entrant found to be, or suspected of: (a) tampering with the entry process or the operation of the Promotion; (b) acting in violation of these Official Rules; or (c) acting in an un-sportsmanlike manner.

Proof of submission of Entry is not considered proof of delivery to or receipt by Sponsor.  Sponsor is not responsible for lost, late, incomplete, inaccurate, stolen, misdirected, undelivered, delayed, garbled or damaged entries; or for lost, interrupted or unavailable network, server, Internet Service Provider (ISP), website, or other connections, availability or accessibility or miscommunications or failed computer, satellite, telephone or cable transmissions, lines, or technical failure or jumbled, scrambled, delayed, or misdirected transmissions or computer hardware or software malfunctions, failures or difficulties, or other errors or difficulties of any kind whether human, mechanical, electronic, computer, network, typographical, printing or otherwise relating to or in connection with the Promotion, including, without limitation, errors or difficulties which may occur in connection with the administration of the Promotion, the processing/reviewing of entries, the announcement of the prizes or in any Promotion-related materials.  Sponsor is also not responsible for any incorrect or inaccurate information, whether caused by Instagram or Twitter users, tampering, hacking, or by any equipment or programming associated with or utilized in the Promotion.  Sponsor is not responsible for injury or damage to participants’ or to any other person’s computer or mobile device related to or resulting from participating in this Promotion or downloading materials from or use of the Instagram or Twitter applications.  Persons who tamper with or abuse any aspect of the Promotion or the Instagram or Twitter applications or who are in violation of these Official Rules, as solely determined by Sponsor and/or Producer, will be disqualified and all associated entries will be void.  Should any portion of the Promotion be, in Sponsor’s or Administrator’s sole opinion, compromised by virus, worms, bugs, non-authorized human intervention or other causes which, in the sole opinion of the Sponsor or Producer, corrupt or impair the administration, security, fairness or proper play, or submission of entries, or should the Promotion be unable to run as planned for any other reason, Sponsor and Producer reserve the right, in their sole discretion to suspend, modify or terminate the Promotion and, if terminated, at its discretion, select the potential winner from all eligible, non-suspect entries received prior to the action taken or as otherwise deemed fair and appropriate by Sponsor.  Sponsor is not responsible for electronic communications that are undeliverable as a result of any form of active or passive filtering of any kind, or insufficient space in Entrant’s email account to receive email messages. CAUTION.  ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THIS PROMOTION IS A VIOLATION OF CRIMINAL AND CIVIL LAWS.  SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT PERMITTED BY LAW.

Choice of Law and Disputes.  This Promotion is only available to legal residents in the United States (excluding Rhode Island, Puerto Rico, territories and possessions), and where otherwise prohibited or restricted by law and subject to all applicable federal, state and local laws and regulations. This Promotion and these Official Rules are governed by and shall be construed in accordance with the substantive laws of the State of California and the United States of America applicable to contracts made and performed entirely in California and where the relevant contacts are within such state.  Any and all legal actions or claims arising in connection with this Promotion must be brought in a court of competent jurisdiction located in Los Angeles, California.  Except where prohibited by law, as a condition of participating in this Promotion, each entrant agrees that any and all disputes, claims and causes of action arising out of or connected with this Promotion, the Prize awarded, or the determination of the winner shall be resolved individually, without resort to any form of class action.  Further, in any such dispute, under no circumstances will an entrant be permitted to obtain awards for, and hereby waives all rights to claim punitive, incidental or consequential damages, or any other damages, including attorneys’ fees, other than entrant’s actual out-pocket expenses, and entrant further waives all rights to have damages multiplied or increased 

Dispute Resolution.  EACH PERSON WHO PARTICIPATES IN THE PROMOTION IRREVOCABLY AGREES THAT SUCH DISPUTE(S) WILL BE RESOLVED SOLELY BY BINDING ARBITRATION BEFORE A SOLE ARBITRATOR, WITH EXPERIENCE IN ADVERTISING OR PROMOTION AND ENTERTAINMENT LAW, UNDER THE RULES AND REGULATIONS OF THE AMERICAN ARBITRATION ASSOCIATION (“AAA”); PROVIDED, HOWEVER, THAT NOTWITHSTANDING THE PARTIES’ DECISION TO RESOLVE ANY AND ALL DISPUTES ARISING UNDER THIS PROMOTION THROUGH ARBITRATION, SPONSOR MAY (1) SEEK TO OBTAIN INJUNCTIVE RELIEF OR OTHER EQUITABLE RELIEF FROM A COURT TO ENFORCE THE PROVISIONS OF THESE OFFICIAL RULES; (2) BRING AN ACTION IN COURT TO PROTECT AND INTERPRET SPONSOR’S INTELLECTUAL PROPERTY RIGHTS; AND/OR (3) BRING AN ACTION TO ENFORCE THE DECISION OF THE ARBITRATOR BEFORE ANY COURT WITH APPLICABLE JURISDICTION.  The arbitration will be held in Los Angeles, California.  The arbitrator will apply the substantive laws of the State of California, will issue a written decision and will have the power to award any legal remedies except as limited by these Official Rules.  The parties agree to split the arbitrator’s fee; provided, however, if applicable law requires Sponsor to pay the arbitrator’s fee in order for the arbitration provision to be enforceable, Sponsor shall have the discretion to elect to pay such fees and proceed to arbitration.  Entrant irrevocably waives any right to bring or join in any class action regarding the Promotion.  Judgment upon the arbitration award may be entered in any court having jurisdiction thereof.  This arbitration provision shall be deemed to be self-executing, and in the event that any party fails to appear to any properly noticed arbitration proceeding, an award may be entered against such party notwithstanding said failure to appear.  In no event shall any entrant seek or be entitled to rescission, injunctive or other equitable relief or to enjoin or restrain the operation of this Promotion (or any web site connected therewith), exploitation of any advertising or other materials issued in connection therewith, or exploitation of this Promotion (or any content or other materials used or displayed on the Web Site) or the entrant’s entry.

Conditions of Participation. By participating in this Promotion, each entrant accepts the conditions stated in the Official Rules, agrees to be bound by the decisions of the judges and warrants that he or she is eligible to participate in this Promotion. Winning a prize is contingent upon fulfilling all requirements set forth herein.  By participating, entrants hereby release and agree to hold harmless Blaze Pizza, LLC, and each of its directors, employees, officers, and agents, including without limitation, their advertising and promotion agencies (collectively, the “Released Parties”), from any and all liability, loss or damages arising from or in connection with the Promotion, awarding, receipt, and/or use or misuse of a prize or participation in any prize-related activities, including, but not limited to: (a) unauthorized human intervention in the Promotion; (b) technical errors related to computers, servers, providers, or telephone, or network lines; (c) printing errors; (d) lost, late, postage-due, misdirected, or undeliverable mail; (e) errors in the administration of the Promotion or the processing of entries; or (f) injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant’s participation in the Promotion or receipt or use of any prize. Except where prohibited, participation in the Promotion constitutes an entrant’s consent to Sponsor’s use of his/her name, likeness, voice, opinions, biographical information, and state of residence for promotional purposes in any media without further payment or consideration.

Third Party Platforms:  If a Promotion is hosted, administered, or operated on a third party platform, including without limitation a social media platform (e.g. Facebook, Instagram, YouTube, or Twitter) (“Third Party Platform”), then by participating in such Promotion, entrants hereby release and agree to hold harmless the Third Party Platform from any and all liability, loss or damages arising from or in connection with the awarding, receipt, and/or use or misuse of prize or participation in any prize-related activities.  Unless otherwise stated to the contrary in the Specific Rules, a Promotion is not sponsored endorsed or administered by, or associated with, any Third Party Platform.

Privacy Policy:  Entrants may receive email correspondence from, or on behalf of Sponsor, subject to the terms of Sponsor’s privacy policy (www.blazepizza.com/privacy). Any questions regarding privacy matters or other important information regarding the collection, use and disclosure of personal information by Sponsor should be directed to the address set forth in the Official Rules.

Results:  To request a list of all winners in this Promotion, please send a self-addressed, stamped envelope to Sleigh Your Cravings Sweepstakes Winners List, Blaze Pizza, LLC, 35 N. Lake Ave., Suite 810, Pasadena, CA 91101.  Requests must be received within four (4) weeks of the close of the Promotion Period.  For a copy of these Official Rules, send a legal-size, self-addressed, stamped envelope to: #BlazeHalloweenPromo Official Rules, Blaze Pizza, LLC, 35 N. Lake Ave., Suite 810, Pasadena, CA 91101 prior to the close of the Promotion Period.  Vermont residents may omit return postage with Official Rules requests and lists of winners of the Promotion.

Miscellaneous:  Sponsor is not responsible for late, lost, stolen, damaged, garbled, incomplete, misaddressed, postage due, or misdirected entries, mail or communications, for errors, omissions, interruptions, deletions, defects, or delays in operations or transmission of information, in each case whether arising by way of technical or other failures or malfunctions of computer hardware, software, communications devices, or transmission lines or data corruption, theft, destruction, unauthorized access to or alteration of entry materials, loss or otherwise.  Further, Sponsor is not responsible for electronic communications which are undeliverable as a result of any form of active or passive filtering of any kind, or insufficient space in Entrant's email account to receive email messages.  The use of automated software or computer programs to enroll or to enter the Promotion is prohibited and any individual who uses or attempts to use such methods to enroll or to enter will be disqualified.  Sponsor disclaims any liability for damage to any computer system resulting from participation in, or accessing or downloading information in connection with, this Promotion, and reserves the right, at its sole discretion, to modify, cancel, terminate or suspend this Promotion should any virus, bug, technical failures, unauthorized human intervention or other causes beyond Sponsor’s control corrupt or affect the administration, security, fairness, integrity or proper conduct of the Promotion. In the event of any such cancellation, termination or suspension, a notice will be posted and a random drawing will be held from among all eligible, non-suspect entries received prior to such time.  Sponsor reserves the right, in its sole discretion, to disqualify any Entrant (and all of his/her Promotion entries) from this Promotion or any other promotion conducted now or in the future by Sponsor or any of its affiliates if he or she tampers with the entry process or if his or her fraud or misconduct affects the integrity of the Promotion.  Sponsors reserve the right to correct clerical or typographical errors in promotional materials. Any attempt by any person to undermine the legitimate operation of the Promotion may be a violation of criminal and civil law, and, should such an attempt be made, Sponsor reserves the right to seek damages from any such person to the fullest extent permitted by law.  Failure by Sponsor to enforce any term of these Official Rules shall not constitute a waiver of that provision.  Proof of sending any communication to Sponsor by mail shall not be deemed proof of receipt of that communication by Sponsor.  The names of individuals, groups, companies, products and services mentioned herein, and any corresponding likenesses, logos and images thereof reproduced herein, have been used for identification purposes only and may be the copyrighted properties and trademarks of their respective owners. The mention of any individual, group or company (including, without limitation, Instagram, YouTube, Facebook or Twitter), or the inclusion of a product or service as a Prize, does not imply any association with or endorsement by such individual, group or company or the manufacturer or distributor of such product or service and, except as otherwise indicated, no association or endorsement is intended or should be inferred. To the extent there is a conflict between these Official Rules and any terms of use or other agreement posted on the Instagram, YouTube, Facebook or Twitter services, these Official Rules shall control.
